Consent for Direct-to-Consumer Laboratory Testing


This document is the Consent Form for Direct-to-Consumer (DTC) Laboratory Testing conducted by Vitality Express Labs, a service of Vitality Urgent Care, and processed by our CLIA-certified reference laboratory partners, Quest Diagnostics and Labcorp. DTC laboratory testing means tests you select and purchase yourself, without a referral from your own healthcare provider. All tests are ordered under the authority of a licensed provider affiliated with Vitality Express Labs, as required by law.

Eligibility: DTC testing is available only to individuals 18 years of age or older.

I understand that the purpose of this testing is to provide me with health information that may be useful in managing my personal health. I understand that a laboratory result by itself is not a medical diagnosis; results require interpretation in the context of my health history, symptoms, and examination, and no test is perfect — false positive and false negative results can occur. I recognize that DTC testing does not replace consultation with a healthcare provider.

Specimen Collection: I consent to the collection of a blood, urine, saliva, or swab specimen as required for the tests I selected. I understand blood collection may cause temporary discomfort, bruising, or soreness, and rarely fainting or infection.

Results: My results will be delivered to me through secure portal or email notification, typically within 3-5 business days of specimen receipt.

Critical Results: If a result is significantly outside the normal range and may require prompt attention, a Vitality Express Labs provider will attempt to contact me directly using the phone number I provide, and may advise me to seek immediate care. I agree to provide accurate contact information for this purpose.

Privacy and Confidentiality: I understand that my information is protected as described in the Vitality Urgent Care Notice of Privacy Practices. I understand that Illinois law requires laboratories and providers to report positive results for certain communicable diseases — including sexually transmitted infections — to public health authorities, and that this reporting is required by law and not optional. If my selected panel includes an HIV test, I consent to HIV testing, and I understand my result is confidential under the Illinois AIDS Confidentiality Act (410 ILCS 305).

Payment: DTC testing is self-pay. No claim is submitted to insurance, and these charges do not count toward my deductible. HSA and FSA cards are accepted. Fees are due at the time of order and are refundable only before my specimen is collected.

Follow-up Care: I understand that DTC laboratory testing does not replace regular medical care, and that further examination may be necessary based on my results. I understand the value of discussing my results with my healthcare provider for interpretation and guidance. If I would like help understanding my results, Vitality Express Labs can arrange an optional, fee-based consultation with a licensed provider at Vitality Urgent Care, our affiliated clinic. Consultation is separate from laboratory testing and entirely my choice.

I have read this Consent Form for Direct-to-Consumer Laboratory Testing, I have had the opportunity to ask questions, and my questions have been answered to my satisfaction. By signing, I freely and voluntarily consent to laboratory testing by Vitality Express Labs, processed by Quest Diagnostics or Labcorp.
